The fake Simpsons prediction scenes appeared in a video (archived here) posted by @simpsonsaidthat on TikTok on July 30, 2025. The post was captioned:

The video opens with a narrator saying:
Trump would be the victim of a heart attack in public during the first week of August 2025, and the Simpsons predicted it years earlier. In an old forgotten episode, we see Trump giving a speech in New York. At first he seems confident but suddenly his voice breaks, he clutches his chest and starts sweating profusely. Lisa, watching on TV, says, "This doesn't look good."
The TikTok video does not show real scenes or a real storyline from The Simpsons.
The Simpsons Wiki Fandom features a synopsis of all the episodes from forty seasons of the show. A search on the site for the terms, 'Donald Trump Heart Attack' (archived here) did not produce any relevant results. The AI-generated content detection tool at Hive Moderation determined with 99.7 percent confidence (pictured below) that the image was AI-generated.

Another edition of this fake Simpsons prediction was posted July 31, 2025 by the @simpson_spredicts YouTube channel (archived here). It features an almost identical transcript but different AI-generated visuals mimicking the show's style (pictured below). A disclaimer required by YouTube was posted with the video stating:
